For such purposes you shall be required to provide mandatory information like (i) date of birth, gender, residential address, marital status, email address, Mobile Number etc. ii) information pertaining to your medical condition such as medical records, family history, prescriptions, past surgeries and lifestyle choices including smoking habits, food preferences (Vegetarian/Non-Vegetarian) etc. and (iii) information sought by The Mynd Project Partners either via online chats, telephone or any other means to offer their Services. The Mynd Project shall collect information based on your interaction with The Mynd Project Partners and access through the aforesaid means which helps The Mynd Project in providing personalized and effective service [Office1] (collectively referred to as “personal data[L2] ”). The Mynd Project shall also collect supplemental information via Cookies, Web Beacons, The Mynd Project mobile device applications and from external sources. (“non-personal data” or “NPD”)

The Mynd Project is not a licensed health care provider, nor should the products and/or services or any part of them, nor any content, including without limitation, text, copy, audio, video, photographs, illustrations, graphics, and other visuals, constitute or be considered a substitute for professional medical advice, diagnosis, treatment or recommendations of any kind. All products, services and content are for informational and educational purposes only and the services and content should not be used during a medical emergency or for the diagnosis, treatment, cure, mitigation, or prevention of any medical condition. Please always consult with a licensed physician or other qualified healthcare providers before making any decisions or taking any actions that may affect your health via the use of the platform, products and our services. We make no representation or warranty about the accuracy, completeness, or suitability for any purpose of the app, any of the products and services, the third-party device, or any advice or information provided as part of our services.

We are not responsible to you or anybody else for any loss incurred in relation to use of the platform, products and services. Your use of the platform, products and any service is entirely at your own risk. We do not promise that the functional aspects of the platform or our content will be error free or that the platform, our content or the server that makes it available are free of viruses or other harmful components. We always recommend that all users of the internet ensure they have up to date virus checking software installed.

Any health information and links on the content, whether provided by The Mynd Project or by contract from outside providers, is provided solely for educational and informational purposes.

You acknowledge that when you access a link that leaves our platform, the site you will enter into is not controlled by us and different terms of service and privacy policy may apply. By accessing links to other sites, you acknowledge that we are not responsible for those sites. We reserve the right to disable links to and / or from thirdparty sites to our service, although we are under no obligation to do so.

BETA SERVICES: We may, from time to time, offer access to products and/or services that are classified as private or public beta versions (“Beta Versions”). Beta Versions are offered “as-is” to allow testing, evaluation and improvement. We make no representations that a Beta Version will ever be made generally available and reserve our right to discontinue or modify a Beta Version at any time without notice, at our sole discretion. Any data you provide to Beta Versions may not be secured and may be lost or damaged, and results provided by certain Beta Versions may be subject to third party licenses, including open source licenses. Any third-party software, services, or other products you use in connection with the Beta Versions are subject to their own terms, and we are not responsible for third party products. We may change our prices for Beta Versions by posting notice to your account and/or to our website and such price increases will be effective immediately.

Data Collection and Management

Purposes for which the personal data is to be processed

The Mynd Project requires personal data of the users for personalized and pertinent health advice and supporting doctor-patient relationship. It also requires personal data for providing adequate Products and Services as defined under the Terms of Use including but not limited to:

Administering account of the user and provide the user with access to particular tools, Products and Services personalized by the user

For validating eligibility for and participation in the Services and Treatment Plans offered by The Mynd Project (i.e., you meet the clinical enrolment criteria for the Services, which may identify those individuals at risk for certain chronic diseases or living with certain chronic diseases)

Sending personalized emails or secure electronic messages pertaining to your health interests, including news, announcements, reminders and opportunities, and provide you with more relevant content and advertisements

Reviewing and enhancing the quality of The Mynd Project Products and Services, and improving your overall experience

To investigate or defend against any allegation or claim against The Mynd Project

Nature and categories of personal data being collected;

Mandatory identifiable information: Name, date of birth, gender, residential address, marital status etc

Contact information: Mobile number, email address, communication address etc

Health data related to the state of physical or mental health of the user and includes records regarding the past, present or future state of the health of user, data collected in the course of registration for, or provision of health services and includes medical records, family history, prescriptions, diagnostic test reports, lifestyle choices etc.

The user may exercise his right to withdraw his consent from permitting The Mynd Project from processing or sharing personal data of the user by giving notice of withdrawal to The Mynd Project at the support@corahealthcare.in. The Mynd Project shall not process or share any personal data of the user once The Mynd Project receives such notice from the user. However, The Mynd Project shall not be liable for any information which is already processed or shared before such notice of withdrawal is given by the user. The Mynd Project also reserves its right to terminate the Account of the user on basis of such notice of withdrawal.

Source of collection of personal data other than the Platform The Mynd Project collects personal data from the users through its Platforms either by proactively asking the user or automatically through Content uploaded by the user on the Platforms. However, The Mynd Project also reserves its right to collect personal data from third parties like social media aggregators, insurance companies, laboratories, medical practitioners etc. The Mynd Project may also collect information from you through phone, SMS or email. You also provide data when you search, buy, post, participate in the Services and Treatment of The Mynd Project or communicate with customer service. As a result of those actions, you might supply us with such information as: your name; address and phone number; credit card information; the personal description in your profile; and your health information. The source of information could be such information which is provided by you, automatically provided via uploading of Content on the Platforms or data received through third parties.

Merger, Sale, Bankruptcy, or Other even

The Mynd Project may sell or buy assets, subsidiaries, or business units. In such transactions, customer information generally is one of the transferred business assets. If we are acquire or merge with, any other entity, we reserve the right to assign or transfer any personal data that we have collected; and, for any bankruptcy, insolvency, reorganization, receivership, or assignment for the benefit of creditors, or for any application of laws or equitable principles affecting creditor’s rights, personal data might be transferred or disclosed to third parties. In case of sharing of personal data on account of the reasons set out in this Clause, users will be provided with a notice of such merger, sale, bankruptcy or any other similar event.

The Mynd Project in course of providing Products and Services may be required to divulge personal data to The Mynd Project Partners. Such personal data has been shared with these third parties in consonance with clause 1 (e). However, The Mynd Project may not have control over the personal data which comes within the knowledge of such The Mynd Project Partners. The Mynd Project does not take any responsibility for any unauthorized usage or sharing of such personal data by third parties. However, The Mynd Project shall assist the user in every way possible in initiating any action against the concerned The Mynd Project Partners for unauthorized usage of personal data of the user.

We partner with Microsoft Clarity and Microsoft Advertising to capture how you use and interact with our website through behavioral metrics, heatmaps, and session replay to improve and market our products/services. Website usage data is captured using first and third-party cookies and other tracking technologies to determine the popularity of products/services and online activity. The Mynd Project encrypts data transmitted to and from the App and the Site[Office5] . Despite The Mynd Project’s efforts to protect your personally identifiable information and personal health information, secure transmission of information via the internet cannot be guaranteed due to security threats outside The Mynd Project’s control and you acknowledge that transmission is at your own risk. By submitting your personal data, you agree to such transmission. The Mynd Project will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy. However, it is made clear that the access, use, transaction, transmission, etc. are done by user at their own volition and risk, and The Mynd Project shall not be responsible and liable in any manner whatsoever.
